Access
The practice premises have suitable access for disabled patients who may register from nearby areas. We have a disabled ramp, a disability compliant reception desk and disabled toilet facilities. There is also a hearing induction loop system on site for those patients who wear hearing aids.

Horton PCN Enhanced Access Service - Information for Patients
This practice is part of the enhanced access service providing GP, Advanced Nurse Practitioner, Nurse, and Health care assistant appointments from the following Hub locations in Bradford:

- Little Horton Lane Medical Centre, 392 Little Horton Lane, Bradford, BD5 0NX
- Oak Lane Surgery, Westbourne Green C H C Centre, 50 Heaton Road, Bradford, BD8 8RA
As a patient registered with this practice, you may have access to the following:
Weekday evening appointments (6.30pm to 9.30pm) and Saturday appointments (9:00 am to 5:00pm) at any of the three hubs (listed above) to see one of the following, subject to availability:
- GP/ANP
- Nurse
- Health care assistant
The service is provided by Horton PCN 6. Medical appointments are with GPs and other healthcare staff who work in the area; so your appointment may not be with a Clinician from this practice.
To use the service, we will ask for your consent to share your medical record, and our reception staff will explain what this means and why it is necessary.

If you need advice out of surgery hours please contact the NHS 111 Service on 111. You can speak in confidence to a Healthcare Professional for advice on a current health problem or obtain information on any NHS service.
